Admission Process

If you or someone you know requires long-term care services, you should contact your local Home Care Coordinator. The Coordinator will arrange a visit with you to assess your personal care needs and if you or someone you know requires long-term care services, you should contact your local Home Care Coordinator. The Coordinator will arrange a visit with you to assess your personal care needs and if appropriate will present your application to the Assessment Panel. If the Panel decides that you meet the eligibility criteria, your application is then forwarded to the Personal Care Home (PCH) of your choice. If the PCH is able to meet your personal care needs, your name is placed on the waiting list for admission.
